2009-2010 School Year Daily Fee Schedule & Registration Process

REGISTRATION FOR SCHOOL YEAR 2009-2010 WILL BEGIN ON MAY 1ST FOR NEW FAMILIES. RETURNING FAMILIES CAN REGISTER BEGINING APRIL 8TH.

  Medication Administration: If your child will need medication to be stored on site a medication authorization form completed by a doctor is required. Medications must be given to staff in the original pharmacy container and be labeled with the child's name, name of medication, directions for medication administration and the date of the prescription.

SPECIAL OFFER: Enroll full time 5 days am & pm and receive a complimentary individual program membership to the YMCA or family program membership if enrolling two or more children. Offer valid with continuous full time (5 days am & pm) enrollment.

TO REGISTER:

1. Complete the reservation request section.
2. Return reservation request with a deposit equal to two-weeks fees (based on the # of days enrolled). Deposit is applied to the last two weeks enrollment in current school year. Mail to YMCA, 99 Union Street, Middletown CT 06457 or drop off at the YMCA.
3. Cancellations prior to the first day of enrollment are subject to a $50 processing fee. Cancellations or changes in enrollment must be submitted in writing two weeks in advance, or security deposit will be forfeited.
4. Membership: YMCA membership is required to participate in Kids’ Korner. Membership may be paid monthly or annually. Monthly fees are $6.50 (individual) and $9.00 (family). Annual fees are $65 plus a $10 enrollment fee (individual) and $90 plus a $10 enrollment fee (family).

Thanks to the United Way and the generous donations of Friends of the YMCA the YMCA Open Doors Program provides assistance to families who earn under $45,000 a year. For information please call 347-6907 or 343-6218.
